Meet Lorna Van Ackeren, BA, MPA

Certified Geriatric Care Manager


I grew up in a close-knit family and learned at a very young age to respect the older adults in my life. This early lesson sparked my journey as an advocate for older people. From a young age, I volunteered at skilled nursing facilities in our neighborhood, and as I got older, I became a friendly visitor for seniors who were socially isolated. When it was time to decide my educational future, it made perfect sense to pursue a degree in Social Welfare and Gerontology.

Alongside my academic and professional achievements, I bring a unique blend of experience, passion, and empathy to support families in their caregiving journey. As an adult child who navigated the complexities of our healthcare system for my aging parents, I deeply understand the challenges families face. This personal journey underscored the vital role of geriatric care management in improving the quality of life for our aging loved ones.

I believe it is critical to help our loved ones age with dignity and respect wherever they call home. During my lifetime in Contra Costa County, I have been incredibly active in many programs that serve our older community: serving as an appointed member of the Contra Costa County Advisory Council on Aging, where I work towards making our hometowns age-friendly communities where people can age in place; as an Executive Board Member for Choice in Aging, I help ensure that these beneficial programs continue to serve our seniors; volunteering as a Commissioner for the Commission on Aging in the City of Pleasant Hill, I give back to the city where I was born and raised; volunteering as a Board Member for Meals on Wheels Contra Costa, I act on my dedication to ensuring that nutritious meals reach the older adults who need them the most; as a member of the East Bay Veterans Community Partnership, which is dedicated to creating a supportive environment for veterans and their families; and as a member of the California Senior Legislature, advocating for the rights and well-being of the elderly population at the state level.

As a Certified Geriatric Care Manager at Aging Care Concierge, I use my extensive knowledge and expertise to help families navigate the challenges of growing older in a complex healthcare system. Acting as a "professional relative," I aim to reduce family caregiver burnout and manage the care of your loved ones with dignity and respect. My goal is to guide families in making informed care choices during this journey, and it would be my honor to be your guide!
