When my friend suffered a massive stroke while I was out of town, I was connected with Lorna Van Ackeren, Certified Geriatric Care Manager — and five months later, I can wholeheartedly add my own glowing reference to the ones that introduced her.
Lorna stepped in immediately, putting in countless hours navigating a complex healthcare situation with nothing but genuine concern for her client. She communicates clearly, makes sound recommendations, and brings warmth and humor to even the hardest circumstances. With her guidance, my friend was relocated to a board and care facility where she's thriving. Lorna manages everything: doctors, insurance, medications, bills, personal shopping, companionship, and ongoing monitoring of her well-being.
She is exactly the watchful, capable professional I'd want in my corner. An absolute treasure.
My commitment to older adults began at an early age. Growing up in a close-knit family, I was taught to respect and value the older adults in my life, and that early influence became the foundation of a lifelong passion for advocating for seniors and supporting them as they navigate the challenges of aging. As a young person, I volunteered at skilled nursing facilities in my community and later became a friendly visitor to older adults experiencing social isolation. When it came time to choose my educational path, pursuing degrees in Social Welfare and Gerontology felt like a natural extension of the work I had already begun.
In addition to my academic and professional experience, I bring something equally important to my work: personal experience navigating the healthcare system as an adult child caring for aging parents. I understand firsthand how overwhelming it can be for families to coordinate medical care, understand healthcare options, manage changing needs, and make difficult decisions while trying to honor the wishes and independence of someone they love. My own experience reinforced my belief in the tremendous value of professional geriatric care management — families should not have to navigate this complicated journey alone.
I believe every older adult deserves the opportunity to age with dignity, respect, safety, and purpose, wherever they call home. Throughout my life in Contra Costa County, I have remained actively involved in organizations and programs dedicated to improving the lives of older adults and their families:
As a Certified Geriatric Care Manager and the founder of Aging Care Concierge, I bring together my professional expertise, community knowledge, and personal understanding of family caregiving to help families navigate the complexities of aging. I often describe my role as being a "professional relative." I become an advocate, coordinator, problem-solver, and trusted resource for both the older adult and the family.
My goal is to help families make informed decisions, coordinate the right resources and services, reduce caregiver stress, and ensure their loved one receives thoughtful, compassionate care. Most importantly, I want every family I work with to know that they do not have to navigate this journey alone. It would be my privilege to become part of your care team and help guide you and your family toward greater peace of mind, dignity, safety, and quality of life.
